During your adventure in Baldur’s Gate 3, you will come across a secret basement in the Blighted Village Apothecary. However, in order to open this secret passage, you must complete a few tasks before entering Ilyn Toth’s lair. Ilyn Toth is not a living character and is only mentioned in his journal, which is not that easy to find.
The basement in the pharmacy is behind a counter in the shop. The entrance can be tricky to find, but once you get close you should see the outline of a wooden hatch light up. If you want to know the exact location, the secret entrance is to the basement X:32 Y:379. Once you find the secret passage, go down to the basement where you will find a bookshelf.
How to move the bookshelf in the Apothecary Cellar
Once you head down into the basement, you’ll find several potions and herbs to loot. Among the loot there will also be one Doctor’s protocol To the left; Make sure you keep it handy for later use. Go to the bookshelf. However, you cannot move it normally or perform an investigative check. However, if you try successfully, you will find some scratch marks on the floor indicating that the bookshelf can be opened. Unless you have a strong companion in your party, you’ll need to look for a lever that will open the bookshelf.
To the left of the bookshelf is a stack of boxes that you can throw. Remove the crate at the top, revealing the lever to open the bookshelf. Use the lever to reveal a secret passage that leads further into the cave.

Once you’ve eliminated the undead enemies, keep going and you’ll see an ornate mirror to the east. As you interact with the mirror, you will be asked a few questions.
